Sports Physicals:

Please have part 1 and 2 completed with parents before arriving at your appointment. MCSD Sports Physical paperwork is required for a sports physical and must be completed before tryouts.

Appointments can be made with your primary care physician or with the Upper Keys AHEC nurse practitioner.
